Release Statement In entering this tournament, I, (we) hereby release Pike County Tennis Association, Pikeville College, City of Pikeville, City of Prestonsburg and any other sponsors from any and all responsibilities for accidents or losses incurred at the location of the tournament or traveling to and from the tournament. I give permission to Pikeville College and the Pike County Tennis Association to use my likeness or voice recording in various forms of media which may include but are not limited to newspaper, television, radio and internet.
